Case Manager / Qualified Mental Health Specialist (QMHS)
Sincere Counseling & Treatment Centers (SCTC) is seeking a compassionate and organized Case Manager/Qualified Mental Health Specialist (QMHS) to join our growing behavioral health team.
This position provides case management and community psychiatric supportive treatment services to children, adolescents, and families while working collaboratively with clinical staff, referral sources, schools, and community providers.
Responsibilities
- Coordinate and monitor client services and treatment goals
- Complete assessments, service plans, and progress documentation
- Provide community-based case management and supportive services
- Coordinate care with schools, hospitals, courts, child welfare agencies, and other community partners
- Maintain timely and accurate documentation in accordance with OhioMHAS and Medicaid requirements
- Participate in treatment team meetings and case consultations
- Assist clients and families with accessing community resources and benefits
- Provide crisis support and follow-up as needed
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related behavioral health field
- QMHS certification or eligibility preferred
- Valid Ohio driver's license and reliable transportation
- Excellent organizational and communication skills
- Ability to work independently while managing multiple cases
- Experience with behavioral health, case management, or community mental health preferred
