TriSource is seeking dedicated Early Intervention Service Coordinators to support families with children (birth to age 3) in accessing essential developmental services within the Philadelphia, PA area.
In this role, you will work closely with families, therapists, and providers to coordinate care, develop service plans, and ensure children receive the resources they need to thrive.
Schedule: Monday - Friday 8:30am-5pm
Requirements:
- Bachelor’s degree in Social Work, Psychology, Child Development, or related field
- Minimum 1 year of experience working with children, families, or individuals with disabilities
- Experience in early intervention, home/community-based services, or case management
- DHS FBI clearance within the past 30 days
- Child Abuse Clearance within the past 30 days
- Ability to pass a background check
Responsibilities
- Coordinate initial and ongoing evaluations for early intervention eligibility
- Develop and manage Individualized Family Service Plans (IFSPs)
- Connect families with appropriate services, supports, and community resources
- Facilitate meetings and collaborate with families and service providers
- Ensure timely documentation and case updates in required systems
- Monitor service delivery and support families throughout the process
- Assist with transition planning as children age out of the program
