Institute on Aging is looking for a Care Manager II to join our CalAIM team, supporting older adults and adults with disabilities so they can live safely and independently in their own homes.
This role is based in San Bernardino County, and candidates must be willing travel to serve clients in the High Desert region. Bilingual Spanish speakers are strongly preferred, as this will help you connect more deeply with the communities we serve.

Your Role: Care Manager
Join our team as a Care Manager, where you'll assess and support clients' psychosocial, physical, mental, and environmental needs. You will develop client-centered care plans, conduct home visits, and facilitate access to community services. This role involves collaborating with clients, caregivers, and healthcare providers, ensuring timely and appropriate care. You will document case management activities, provide crisis intervention, advocate for clients, and participate in ongoing quality improvement. A strong understanding of Medicare and Medi-Cal is highly preferred.
What You'll Do
- Conduct comprehensive assessments of clients' medical, psychosocial, mental health, environmental, and spiritual needs
- Build and continually update client-centered Community Living Plans
- Make home visits, hospital and skilled nursing facility visits, and accompany clients to medical appointments as needed
- Connect clients to community resources using your knowledge of Medicare, Medi-Cal, and other entitlement programs
- Provide crisis intervention, advocacy, and therapeutic support
- Maintain clear, timely documentation and collaborate closely with your care team
- Continuously evaluate whether clients can safely remain at home and coordinate alternative placement when needed
What You'll Bring
- M.S.W. or another relevant Master's degree (MPH, MFT, etc.), OR a BA/BS in Social Work or related field with 3+ years of relevant social work experience. Required
- At least one year of experience working with older adults and/or adults with disabilities. Required
- Strong assessment, case management, and problem-solving skills
- Excellent communication skills with clients, families, and multidisciplinary health teams
- Comfort with computers; EPIC electronic medical records experience a plus
- Spanish fluency preferred
- Reliable transportation and willingness to travel throughout San Bernardino County, including the High Desert
Physical Requirements
- Communication: Ability to talk and hear during face-to-face communications.
- Mobility: Ability to reach overhead and below the knees, including bending, twisting, pulling, and stooping.
- Coordination: Multi-limb and eye-hand coordination.
- Endurance: Ability to sit at a desk for prolonged periods while working on computers. Ability to move, lift, carry, push, pull, and place objects weighing more than or equal to 40 pounds without assistance.
