Job Overview
Mill City Home Health Care is seeking a compassionate and dedicated Clinical Nurse (RN) to join our assisted living team. Duties
- Conduct comprehensive resident assessments, including health status reviews, nursing evaluations, and ongoing monitoring of resident conditions.
- Develop, review, and update individualized service plans and nursing care plans in accordance with resident needs and regulatory requirements.
- Oversee medication management services and provide guidance to staff regarding medication administration and health-related services.
- Monitor residents for changes in physical, mental, or emotional condition and communicate concerns to physicians, families, case managers, and other healthcare professionals.
- Provide nursing consultation and clinical oversight to direct care staff.
- Participate in resident admissions, discharges, and transitions of care.
- Assist with physician orders, medication reviews, treatment monitoring, and coordination of healthcare appointments.
- Ensure accurate and timely documentation in compliance with HIPAA, Minnesota Department of Health regulations, and facility policies.
- Provide staff education and training on topics such as infection control, medication administration, resident rights, emergency procedures, and chronic disease management.
- Assist with incident investigations, quality assurance activities, and regulatory compliance initiatives.
- Support residents with chronic health conditions, mental health diagnoses, developmental disabilities, and age-related health concerns.
Pay: From $2,000.00 per month
