Qualifications: Required: Possess one of the following: • Masters of Science Degree in Nursing (MSN) from an accredited institution. • Bachelors of Science Degree in Nursing (BSN) from an accredited institution and 5 years of case management or manager care experience. Possess a current license to practice as a Registered Nurse in the State of Ohio. 3 years of case management or managed care experience. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication, and delegation and problem-solving skills. Demonstrates strong analytical, data management, and computer skills. Demonstrates strong leadership, organizational, and time management skills. Reliable transportation to make clinic or home visits to patients as needed. Preferred: Master’s Degree in a healthcare-related field from an accredited institution. Current Certified Case Manager (CCM) by the Commission for Case Manager Certification (CCMC), or other similar case management certification. Experience with case management, access management, home health care, psychiatric case management, discharge planning, or similar related experience. Knowledge of EPIC functionality. Knowledge of case management software. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
