RN/LPN MDS Nurse

Responsibilities:
- Assess, plan, implement, and evaluate patient care in accordance with established standards and regulations
- Conduct MDS assessments to identify patient needs and develop individualized care plans
- Collaborate with interdisciplinary team members to ensure comprehensive care and optimal outcomes
- Develop and maintain effective communication with patients, families, and healthcare team members
- Maintain accurate and timely records of patient care and treatment plans
- Participate in quality improvement initiatives to ensure continuous improvement in patient care
Requirements:
- 1+ year of RN/LPN experience in a post-acute rehabilitation setting
- Current RN/LPN licensure in the state of Louisiana
- Strong clinical skills and knowledge of MDS regulations and guidelines
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
