American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract in the San Mateo, CA area.

Home health setting serving patients across a defined geographic territory including South SF, Pacifica, Millbrae, Hillsborough, Burlingame, San Carlos, Foster City, San Mateo, Redwood City, Emerald Hills, Menlo Park, Mountain View, and neighboring cities

Day shift hours from 8am–5:30pm, with schedule flexibility based on patient availability

13-week contract

Minimum 1 year of home health PT experience required

Current and active state PT license required (license required if applicable)

BLS/BCLS certification required

Responsibilities include evaluating and treating patients using medically prescribed physical therapy programs to restore function and maximize patient performance

May supervise Physical Therapy Assistants as part of care team duties

Responsible for completing initial admission/evaluation visits and reporting OASIS scores to the Clinical Team Manager within established timelines

Documents clinical findings, skilled interventions, goals, and discharge plans in accordance with agency standards
