Home Care by Callos, is a non -skilled Home Care Agency currently recruiting a Registered Nurse professional for the position of Admissions Nurse . THERE IS NO PATIENT CARE INVOLVED IN THIS ROLE. Must be available 15 hours a week, Day shifts. Position is part time 15 hours a week. This position is ideal for a dynamic, self-motivated professional who enjoys being on-the-go and has a passion for making meaningful connections. A $1,000 sign-on bonus will be provided, contingent upon meeting requirements.

Responsibilities of the position include:
- Educate healthcare professionals, facilities, physicians, and community organizations on the benefits and services of Home Care by Callos
- Perform comprehensive nursing assessments for admissions, readmissions, and start of care visits and perform orientations for new caregivers.
- Build and maintain strong professional relationships with Clients and Families.
- Ensure compliance with all regulatory requirements and company policies.
- Must be able to work with waiver/ private pay patients and CareScout clients
- Paid per visit/ plus mileage
- Develop and implement individualized care plans based on patient needs.
- Complete Carescout assessments accurately and timely.
- Remain active and on the go throughout the day educating patients/conducting training, and community partners.

Qualifications / Experience Requirements:
Must have:
- State Licensed RN
- Experience in a Homecare setting preferred.
- Experience working with elderly patients
- Excellent written and verbal communication skills
- Must be tech savvy
- Strong interpersonal, communication, and relationship-building skills.
- Self-starter with excellent organizational and time -management abilities.
- Ability to work independently while being part of a collaborative team
- Valid drivers license, reliable transportation, and current auto insurance
