Pay:$65.00 per point
- Start of Care (SOC): 2 points
- Resumption of Care (ROC) / Recertifications: 1.5 points
- Non-skilled SOC / Evaluations: 1.5 points
- Discharges & Routine Visits: 1 point
- Mileage reimbursement is paid at $0.55 per mile.
- On-Call Compensation: $300 per week for assigned Home Health or Hospice on-call shifts (in addition to standard visit rates)

Summary
We are seeking a compassionate and skilled Registered Nurse to join our team in delivering exceptional healthcare in the comfort of our patients' homes. This role is vital in ensuring that each patient receives tailored care that aligns with our commitment to improving their quality of life.
Responsibilities
- Provide direct patient care and support to individuals in their homes.
- Conduct comprehensive assessments and develop individualized care plans.
- Administer medications and monitor patient responses.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Educate patients and families on health management and wellness strategies.
- Maintain accurate medical records and documentation.
- Ensure compliance with healthcare regulations and standards.
- Participate in ongoing training and professional development.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in home healthcare or similar settings.
- Strong clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a team.
- Knowledge of healthcare regulations and best practices.
- CPR certification required; additional certifications preferred.
Job Type:

- The on-call period runs from Friday at 3:00 PM through the following Friday at 8:00 AM.
