We are seeking a compassionate and experienced Registered Nurse (RN) to join our home health care team that must be willing to see patient's in Harrison and Crawford counties of Indiana.
The ideal candidate will have a strong background in providing skilled nursing care. This role involves coordinating and delivering quality care to patients in their homes, ensuring all services are provided in accordance with physician orders and regulatory standards.
Key Responsibilities:
- Conduct initial and ongoing comprehensive assessments of patients, including OASIS documentation.
- Develop, implement, and evaluate individualized care plans based on patient needs.
- Administer skilled nursing care such as wound care, IV therapy, catheter care, medication administration, and chronic disease management.
- Monitor patient progress and communicate changes to the physician and care team.
- Educate patients and families on disease processes, medication management, and self-care techniques.
- Document all visits and interventions accurately and timely in the electronic medical record (EMR).
- Ensure compliance with all applicable federal, state, and local regulations and company policies.
- Collaborate with interdisciplinary team members to provide holistic and coordinated care.
Pay: $44.00 - $50.00 per hour
