Job Summary
We are seeking a dedicated and highly skilled Wound Care Nurse Practitioner to join our dynamic healthcare team. Responsibilities
- Conduct thorough patient assessments, including wound evaluation, vital signs, and medical history review using electronic health record (EHR) systems such as Epic or eClinicalWorks.
- Develop and implement individualized wound care treatment plans incorporating advanced techniques like suturing, debridement, and dermal fillers when appropriate.
- Administer specialized therapies such as Botulinum toxin treatment for related conditions and manage complex cases involving infectious disease care or acute pain management.
- Collaborate closely with physicians, nurses, physical therapists, and other healthcare professionals to coordinate comprehensive patient care across settings including hospitals, nursing homes, assisted living facilities, and outpatient clinics.
- Perform procedures such as IV infusion, catheterization, tube feeding management, and sterile dressing changes while maintaining strict infection control standards.
- Document all patient interactions accurately in EMR/EHR systems like Cerner or Athenahealth, ensuring compliance with HIPAA regulations and CPT coding requirements.
- Educate patients and families on wound healing processes, self-care techniques, medication administration, and health coaching strategies to promote optimal recovery outcomes.
Qualifications
- Current Nurse Practitioner license with certification in wound care or related specialties preferred; valid DEA registration for prescribing medications.
- Proven experience working as a nurse practitioner in diverse clinical environments such as hospital medicine, critical care (ICU/Level I & II trauma centers), or post-acute care settings.
- Demonstrated expertise in advanced procedures including suturing, phlebotomy, venipuncture, sterile processing techniques, and injection administration.
- Familiarity with EMR/EHR systems such as Epic, Cerner or eClinicalWorks; experience with ICD-10/ICD-9 coding and CPT coding is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology related to wound healing and pain management; experience with telehealth platforms is a plus.
- Critical thinking skills with the ability to perform comprehensive patient assessments and diagnostic evaluations efficiently.
- Excellent client communication skills to effectively educate patients of all ages—including pediatrics (children) and geriatrics—while fostering collaborative practice agreements within multidisciplinary teams.
