We're currently on the lookout for individuals for 1099 or W-2 roles based in or around the vibrant city of Phoenix, AZ. As a clinician-owned and operated company, this opportunity involves evaluating and assessing patients' speech and language conditions, crafting and executing treatment plans, and diligently documenting patient progress. Requirements:
- Graduate from an accredited Speech Therapy program.
- Hold an active or eligible Speech Therapy Licensure in Arizona (required by the start date).
- Maintain current CPR Certification, Valid Fingerprint Clearance Card, Article 9 Training (required upon start date, new-hires eligible to receive support to complete requirements)
- Has completed a minimum of 9 months of experience after being awarded ASHA certification (i.e., completion of the 9-month Clinical Fellowship followed by 9 months of experience)
- Experience in the Arizona home-health setting required
Essential Functions:
- In addition to supervision of SLPA/CF-SLP, Supervising SLP must be able to hold their own caseload to evaluate and treat a diverse caseload, including patients with Autism, Down Syndrome, Cerebral Palsy, Cognitive Disabilities, and other chromosomal syndromes
- Document SOAP notes, progress reports, and evaluation reports using a user-friendly digital EMR system
- Provide appropriate amount of supervision based on the SLPA license requirements
- A licensed SLP may supervise an SLPA via telecommunication technology, as the SLPA provides clinical activities, if the supervising SLP can provide ongoing immediate feedback throughout the clinical activity being provided
Salary:
- 1099 roles: Starting at $90 per visit in-person, eval rate starting at $150/visit in-person
