As an LPN Nurse Manager - Personal Care, you will carry out our Mission of Love by coordinating Health Care Services for residents in the Star Points building and supervision of the staff providing these services.
Our LPN Nurse Manager - Personal Care is responsible for the following:
- Serves as a resource for staff concerning resident care concerns and equipment needs for Personal Care.
- Accountable for assuring an appropriate staffing schedule to meet resident care needs as well as regulatory guidelines.
- Delegates to nursing personnel the administrative authority, responsibility, and accountability to perform their assigned duties.

Monitors and assesses health status of Personal Care residents.
- Assesses residents for appropriate level of care and ensure placement so that the appropriate level of services can be provided.
- Reviews all resident assessments and support plans for accuracy and completion.
- Develops and implements tools to assess and accurately record resident care needs and care delivered by Personal Care staff.

Works closely with staff physicians to coordinate resident care needs.

Ensures that all health care services are in compliance with the Department of Public Welfare regulations.
- Prepares building for survey process as well as providing mandatory items (documentation) for inspection.
- Coordinates transfers to and from the HCC with nursing and social services staff.
- Collaborates with Dietary Services concerning nutritional needs for Personal Care residents.
- Assists in keeping staff educated in geriatric issues through in-services and by attendance at seminars. Trains all CNAs DPW approved medication administration classes and monitors them two (2) times a year.

Conducts reviews and investigations of all incident and accident reports submitted by Personal Care staff.
- Reviews concerns, complaints, etc. from residents, families/responsible parties. Takes necessary steps to resolve issues.
What You Need as an LPN Nurse Manager - Personal Care:

Graduation from an accredited School of Nursing (Practical Nurse). Bachelor of Science degree in Nursing preferred.

Current licensure as a Practical Nurse by the Pennsylvania State Board of Nursing required.

Two years nursing experience required in a long-term care environment and/or Personal Care experience preferred.

Satisfactory experience in a leadership role required.

Current knowledge of state and federal regulations pertaining to long-term/Personal Care care facilities required.

BLS / First Aid instructor required.

Good written, oral, organizational, and basic computer skills
-
CPR certification required.
