Position Summary
We are seeking compassionate and dependable Licensed Vocational Nurses (LVNs) to provide skilled nursing care for patients in their homes during weekend shifts. Under the supervision of a Registered Nurse, you will play a vital role in promoting patient well-being, supporting independence, and ensuring continuity of care.
This position offers the opportunity to build meaningful relationships with patients and families while developing valuable hands-on experience in the home health setting.
Responsibilities
- Provide skilled nursing care in the home setting
- Follow individualized care plans and physician orders
- Monitor, document, and report patient progress and changes in condition
- Administer medications and treatments as prescribed
- Collaborate with the interdisciplinary healthcare team to ensure quality patient care
- Educate patients and families regarding care plans, medications, and disease management
- Maintain accurate and timely clinical documentation
Qualifications
- Current and active California LVN license
- Current CPR/BLS certification
- Current TB test or chest X-ray (if applicable)
- Valid driver's license and reliable transportation
- Strong communication and documentation skills
- Compassionate, dependable, and professional demeanor

New Graduate LVNs Welcome!
At Prime Home Health, we are passionate about supporting new graduates as they begin their nursing careers. We provide a strong foundation, ongoing mentorship, and valuable hands-on experience to help you develop confidence and grow as a healthcare professional.
We offer paid training and opportunities through our Flex Waiver Program to help nurses gain experience across a variety of patient populations. After one year of employment, we can also assist with obtaining tracheostomy and ventilator certifications.
COVID-19 Considerations
Employee and patient safety remain our top priority. We follow strict infection control protocols and provide PPE, including gloves, masks, and face shields, upon request. Hired nurses will not be assigned to COVID-19 positive patients.
To further reduce exposure, our nurses generally provide care for one patient, allowing for greater continuity of care and fewer interactions across multiple homes.
