We are seeking a dedicated and dynamic Long-Term Support Services (LTSS) Specialist to join our healthcare team. Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans that address acute and chronic conditions.
- Manage discharge planning processes by coordinating post-acute care services, ensuring smooth transitions between care settings.
- Utilize medical records and documentation systems to accurately record patient information in accordance with HIPAA regulations.
- Perform diagnostic evaluations and support utilization management efforts to optimize resource use and patient outcomes.
- Engage in case management activities, including intake procedures, patient service coordination, and follow-up care.
- Provide specialized support for diverse populations such as geriatrics, pediatrics, behavioral health patients, and those requiring infectious disease care or trauma services.
Requirements
- Proven experience in critical care environments such as ICU, emergency room, or Level I/II trauma centers.
- Strong background in hospital medicine, medical-surgical units, or outpatient clinics with a focus on acute and post-acute care.
- Knowledge of hospital procedures including medical terminology, patient assessment techniques, and physiology fundamentals.
- Experience working within managed care frameworks and utilizing diagnostic evaluation tools for effective case management.
- Familiarity with HIPAA compliance standards and medical records management systems.
- Excellent communication skills for effective discharge planning, patient education, and interdisciplinary collaboration.
