An established social services organization is seeking a Case Manager in Brooklyn, NY. This role supports residents through assessment, advocacy, service coordination, and ongoing case management in a residential setting.
About the Opportunity:

Shift: Day shift

Schedule: Monday through Friday, with a Saturday rotation and one late day per week

Hours: Standard business hours, with occasional late-day coverage and summer Fridays ending at 3:00 PM as workload permits

Setting: Residential setting
Responsibilities:

Manage a caseload of residents and meet with clients on a weekly and as-needed basis

Connect clients with appropriate providers, programs, and community resources across health and human services settings

Escort clients to scheduled and unscheduled medical and psychiatric appointments when needed

Facilitate group activities, track participation, and support on- and off-site programming

Maintain accurate client records, including progress notes, service plans, assessments, and updates, in an audit-ready format

Conduct unit inspections, support follow-up needs, and ensure safe service delivery in alignment with program and food safety standards
Qualifications:

Bachelor's degree

At least 2 years of experience in human services

Strong spoken and written communication skills

Strong time management skills and the ability to multitask in a fast-paced environment

Ability to build therapeutic relationships with clients and collaborate with internal and external partners

Proficiency with Microsoft Office, email, and internet navigation

Knowledge of confidentiality and HIPAA-related practices
