Overview
We are seeking a dedicated and detail-oriented Part-time MDS (Minimum Data Set) Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation process for residents in long-term care facilities, ensuring compliance with federal and state regulations. Duties
- Conduct thorough MDS assessments for residents, ensuring accuracy and completeness in accordance with CMS (Centers for Medicare & Medicaid Services) guidelines.
- Review medical records, clinical documentation, and care plans to ensure compliance with HIPAA and NCQA standards.
- Collaborate with interdisciplinary teams including nursing, therapy, social work, and physicians to gather comprehensive resident data.
- Utilize EMR (Electronic Medical Record) PCC, to document assessments and update patient records efficiently.
- Perform case management activities including discharge planning, hospice care coordination, and transition of care processes.
- Ensure timely submission of MDS schedules and reports to facilitate Medicare reimbursement and DRG (Diagnosis-Related Group) coding accuracy.
- Maintain knowledge of ICD-10/ICD-9 coding systems, CPT coding, and medical terminology to support precise clinical documentation improvement efforts.
Qualifications
- Proven experience in hospital environments, or nursing homes with a focus on MDS coordination or clinical documentation review.
- Strong understanding of managed care processes, utilization review, utilization management, and health information management standards.
- Familiarity with inpatient/outpatient coding systems including ICD-10/ICD-9/DRG classifications.
- Experience working with EMR/EHR systems such as Point Click Care is preferred.
- Knowledge of physiology, anatomy, nursing practices, and medical terminology essential for accurate assessments.
- Has a working knowledge of MDS and PPS process and able to meet deadlines.
- Excellent organizational skills a must, with the ability to manage multiple priorities while maintaining attention to detail.
