Roanoke Resource is seeking a compassionate, dedicated, and community-focused Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. The ACT RN plays a vital role in supporting individuals living with serious mental illness by providing psychiatric nursing services, coordinating healthcare, promoting medication compliance, and helping clients maintain successful community placements.
Our ACT team works collaboratively to reduce psychiatric hospitalizations, improve quality of life, and empower individuals to achieve greater independence and stability.
Schedule
- Full-Time or Part-time, 20-40 hours per week
- Rotating Schedule
- Some weekend coverage required
- Community-based position requiring travel throughout the service area

Qualifications
Required:
- Current Virginia Registered Nurse (RN) License
- Valid driver's license and reliable vehicle
- Demonstrated basic computer proficiency
- Ability to pass a criminal background check and drug screen
Preferred:
- Minimum one (1) year experience working with individuals diagnosed with Serious Mental Illness (SMI), Intellectual and Developmental Disabilities (IDD), and/or Substance Use Disorders (SUD)
Essential Duties and Responsibilities
Psychiatric Nursing & Clinical Services
- Provide psychiatric nursing services to ACT clients in community settings.
- Complete nursing and psychiatric assessments.
- Monitor psychiatric symptoms and medication effectiveness.
- Assess symptom acuity and coordinate interventions to prevent hospitalization and crisis episodes.
- Participate in treatment planning and interdisciplinary team meetings.
- Conduct home visits and community-based nursing services.
Medication Management
- Conduct medication education for clients and staff.
- Assist individuals with weekly medication bag/box fills.
- Deliver medications to clients' homes as part of scheduled morning and evening medication runs on a rotating basis.
- Administer and monitor psychotropic medications and injections.
- Monitor medication administration records (MARs), medication storage, provider orders, and medication inventory.
- Promote medication compliance and support medication management plans.
- Perform all required medication-related documentation accurately and timely.
Care Coordination & Advocacy
- Coordinate medical and behavioral healthcare services for ACT clients.
- Accompany and advocate for clients during medical appointments when needed.
- Connect clients with community resources and support services.
- Follow up on referrals, diagnostic testing, and treatment recommendations.
Health Education & Preventive Care
- Perform basic health assessments and preventive health screenings.
- Provide education regarding medication compliance, chronic disease management, substance use, tobacco cessation, nutrition, and overall wellness.
- Identify health concerns and coordinate appropriate follow-up care and referrals.
Documentation & Compliance
- Ensure nursing services are documented accurately and in compliance with applicable regulations and standards.
- Review and audit clinical documentation for accuracy, completeness, and timeliness.
- Maintain knowledge of behavioral health regulations, healthcare standards, and compliance requirements.
- Utilize Personal Protective Equipment (PPE) and follow organizational safety protocols.
- Represent Roanoke Resource in a positive manner and perform duties in accordance with the organization's Mission, Code of Conduct, and Ethical Practices.
- Other duties as assigned by supervisor.
