What You'll Do
As our Clinical Liaison, you'll be the face of our facility throughout the community while helping ensure a smooth and efficient admissions process.
Relationship Building & Business Development
- Develop and execute the facility marketing and census development plan.
- Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, clinics, and community organizations.
- Conduct routine hospital and referral source visits to increase awareness of our services.
- Identify opportunities to grow admissions and strengthen community partnerships.
- Represent the facility at networking events, community functions, and healthcare meetings.
Clinical Review & Admissions
- Review referrals and collaborate with the interdisciplinary team to determine admission appropriateness.
- Assist referral sources and families through the admissions process from inquiry to admission.
- Coordinate collection and completion of admission documentation.
- Ensure all admission paperwork is complete and distributed to appropriate departments.
- Maintain accurate records of available beds and admissions activity.
- Manage resident waiting lists in accordance with facility policies.
Customer Service Excellence
- Serve as a trusted resource for patients, families, hospitals, and referral partners.
- Respond quickly and professionally to questions, concerns, and admission requests.
- Help create a positive admissions experience and seamless transition into the facility.
- Participate in resident and family orientation activities.
Census & Regulatory Support
- Maintain daily and monthly census reports.
- Monitor referral trends and provide regular updates to leadership.
- Stay current on Medicare, Medicaid, managed care, and skilled nursing admission requirements.
- Work closely with nursing, therapy, business office, and clinical teams to optimize admissions and resident outcomes.
What We're Looking For
Required Qualifications
- Experience in healthcare marketing, admissions, business development, case management, or community liaison services.
- Strong understanding of post-acute care, skilled nursing, rehabilitation, or healthcare services.
- Excellent communication, relationship-building, and presentation skills.
- Organized, self-motivated, and able to manage multiple priorities.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Passion for serving patients, families, and referral partners.
Preferred Qualifications
- Skilled nursing facility, rehabilitation, home health, hospice, or hospital experience.
- Knowledge of Medicare, Medicaid, and post-acute care regulations.
- Existing relationships within the local healthcare community.
- Clinical background (RN, LPN, PTA, OTA, RT, etc.) preferred but not required.
