JOB SUMMARY: The Primary Care Nurse Manager (PCNM) provides operational and clinical oversight of the agency’s triage and primary care nursing service lines. This role is responsible for optimizing workflows, improving patient experience and advancing quality and operational outcomes through standardized nursing practices and service line development, including monitoring performance metrics, supporting regulatory compliance and quality assurance initiatives. This position partners closely with clinical, operational and quality leadership to improve efficiency, reduce provider burden and align nursing services with organizational and value-based care goals.
This role requires a field-based presence across all three health center sites in Guerneville, Occidental and Sebastopol. During the initial six months, remaining time will primarily focus on program development and administrative responsibilities. Following implementation of key program components, the role will transition to mostly field-based work, a small amount of direct patient panel management, and administrative responsibilities.

RESPONSIBILITIES:
Triage:
- Operational - Oversees initial assessment, prioritization and routing of patients to ensure timely and appropriate care. Includes supervision of staff, management of staffing and scheduling adjustments, workflow oversight and employee performance management.
- Triage Model Continuous Improvement – Will identify current gaps and opportunities to improve triage with an eye towards exploring triage models that improve efficiency, patient access and clinical appropriateness.
- Performance tracking - Metrics for this service line include assessment completion rate, clinical accuracy, average call time, volume handled within critical and non-critical acuities, wait time, triage to provider time and referral rate for urgent care and the ED. The Primary Care Nurse Manager will track metrics and use them for quality control and program improvement.
- Compliance and Quality Assurance – Ensure adherence to state and federal regulations, contractual requirements and maintain accurate documentation in the EHR.
Primary Care:
- Operational – Oversees nurse support visits and clinic nursing work being performed by Primary Care Nurses. Includes hiring, training and supervision of staff, taking sick calls and adjusting staffing, and handling disciplinary issues. Develop, standardize and implement workflows related to nurse support visits, medication administration, wound care, emergency response procedures and crash cart management. Provide training and ongoing competency support to primary care nursing staff across sites.
- Service Line Development – The Primary Care Nurse Manager will support standardization across the sites and continuously evaluate and implement care models that support value-based care initiatives, improve patient outcomes and reduce provider burden.
- Performance Tracking – Metrics for this service line include number of nurse support visits completed by each Primary Care Nurse, cycle time for the nurse support visits and measurable outcomes tied to values-based care. The Primary Care Nurse Manager will track these metrics and use them for quality control and program improvement.
- Compliance and Quality Assurance – Ensure adherence to state and federal regulations, contractual requirements and maintain accurate documentation in the EHR.
- Assist in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies.
- Collaborate with the MA Supervisor, Clinic Manager and Site Medical Director on Infection Control, including monthly sign off on the autoclave duo spore report and participation in the delivery of infection control trainings, as well as staff management and workflow development.
- Develop and implement a Triage Quality Watch and Primary Care Nurse Quality Watch; review completed Quality Watches; develop and implement corrective action plans as needed.
- Initiate variance reports for completion by the Clinic Manager or Site Medical Director.
- Submit reports for Vaccine Adverse Reactions.
- When direct reports are out, the Primary Care Nurse Manager will manage staffing. Per diems will be utilized and, if a per diem is not available, the Primary Care Manager may need to assist with clinic coverage.
REQUIREMENTS:
- Current California Registered Nurse License in good standing.
- Minimum 3-5 years nursing leadership experience.
- Experience in ambulatory care, primary care or triage nursing.
- Knowledge of regulatory and quality standards in outpatient settings.
- Experience with EHR systems and clinical reporting tools.
- Valid California driver’s license and reliable transportation.
- Current CPR certification.
- BSN required / MSN preferred
- Experience in FQHC or value-based care settings
- Bilingual Spanish preferred
PHYSICAL REQUIREMENTS:
- Ability to bend and move arms with sufficient range of motion to complete patient related requirements, may need to assist patients onto exam table, must be able to lift 30 lbs., bend, kneel and twist as needed and hearing is required for patient assessments.
- Prolonged standing and occasional sitting required.
- Must be able to perform the essential duties with or without accommodations. Must possess basic office and equipment safety knowledge and practice.
