Indeed Job Posting: Director of Clinical Services / Director of Nursing – Home Health
Family Home Care Group is seeking an experienced Director of Clinical Services / Director of Nursing to lead our clinical operations and support the continued growth of our home health agency. This role is responsible for overseeing patient care quality, clinical compliance, staff supervision, documentation standards, and day-to-day clinical operations.
The ideal candidate is a strong clinical leader with home health experience, knowledge of Medicare regulations, and the ability to support field clinicians while maintaining high standards of patient care and regulatory compliance.
Position Summary
The Director of Clinical Services / Director of Nursing provides leadership and oversight for all clinical services, including skilled nursing, therapy, home health aide services, care coordination, quality assurance, and regulatory compliance. This position works closely with agency leadership to ensure safe, effective, timely, and patient-centered care.
Key Responsibilities
Oversee clinical operations and ensure care is delivered in accordance with physician orders, the plan of care, agency policies, Medicare Conditions of Participation, AHCA requirements, and applicable accreditation standards.
Supervise and support clinical staff, including RNs, LPNs, PTs, PTAs, OTs, COTAs, SLPs, MSWs, and HHAs as applicable.
Review and monitor clinical documentation for accuracy, timeliness, medical necessity, skilled need, and compliance.
Ensure timely completion and review of OASIS assessments, Plans of Care, recertifications, resumptions of care, discharges, supervisory visits, and physician orders.
Assist with clinical case conferences, care coordination, patient status updates, and communication with physicians, referral sources, patients, caregivers, and field staff.
Monitor patient outcomes, hospitalization trends, missed visits, visit utilization, infection control, and other quality indicators.
Participate in QAPI activities, performance improvement initiatives, clinical audits, and regulatory survey readiness.
Provide clinical guidance, education, and support to field clinicians regarding documentation, patient care standards, OASIS accuracy, care planning, and regulatory compliance.
Assist with hiring, onboarding, competency validation, performance evaluations, coaching, and corrective action for clinical staff.
Ensure appropriate staffing, visit coverage, and coordination between intake, scheduling, clinical, and billing departments.
Review high-risk patients, changes in condition, incidents, complaints, and care concerns to ensure appropriate follow-up and resolution.
Maintain compliance with agency policies, state and federal regulations, Medicare standards, and accreditation requirements.
Promote a positive, professional, team-oriented culture focused on quality care, accountability, and excellent patient service.
Qualifications
Current Florida RN license required.
Minimum of 2 years of home health experience strongly preferred.
Prior leadership, supervisory, DON, Clinical Manager, or Director of Clinical Services experience preferred.
Strong understanding of Medicare home health regulations, OASIS, Plans of Care, skilled need, documentation standards, and survey readiness.
Knowledge of AHCA, CMS Conditions of Participation, infection control, QAPI, and clinical compliance expectations.
Excellent leadership, communication, organizational, and problem-solving skills.
Ability to coach clinicians, review documentation, manage priorities, and work collaboratively with agency leadership.
Comfortable using EMR systems and reviewing clinical documentation electronically.
Preferred Experience
Home health leadership experience.
OASIS review experience.
Survey or accreditation readiness experience.
QAPI participation or leadership.
Experience supervising both nursing and therapy disciplines.
Schedule
Full-time position.
Monday through Friday, with availability as needed for clinical leadership responsibilities.
