Full-time Physical Therapist position in South of San Jose, CA, supporting a multidisciplinary team to help patients regain mobility and independence. The role offers opportunities for professional development and career advancement.
Evaluate patients and develop individualized treatment plans based on physician orders and patient goals.
Perform comprehensive assessments and administer evidence‑based interventions.
Develop care plans that promote functional mobility, strength, balance, gait, endurance, and overall independence.
Provide high‑quality, safe, and compassionate care across appropriate age groups.
Complete accurate and timely documentation for evaluations, treatment sessions, progress notes, and discharge planning.
Assign and supervise PTAs and rehabilitation aides to support efficient patient care.
Review and update treatment plans for patients receiving services from support staff.
Educate patients, families, caregivers, and healthcare staff regarding treatment plans, home exercise programs, and injury prevention.
Assist in the orientation of new employees and volunteers.
Supervise and evaluate physical therapy students and Physical Therapist Assistant students during clinical affiliations.
Assume Charge Therapist responsibilities as needed.
Coordinate care with physicians, nurses, case managers, and other rehabilitation professionals.
Collaborate with California Children’s Services case managers to ensure continuity of care following discharge.
Adhere to all safety policies, regulatory standards, and professional practice guidelines.
Perform additional duties as assigned.
