$65-$85/hour (all inclusive)
At VHS Hospice, we believe that compassionate, expert clinical care is essential to supporting patients and families through serious illness and end-of-life care. Guided by our core values of Passion, Respect, and Excellence, the Nurse Practitioner provides advanced clinical expertise in hospice and palliative care, ensuring comfort, dignity, and individualized support for patients and their loved ones.
Responsibilities for Nurse Practitioner:
Provide medical support and coordination to hospice patients and family members.
Provide medical expertise in pain and symptom management and evaluate terminal prognosis for patients, families, and staff.
Ensure care delivery aligns with established hospice and palliative care guidelines, standards, and best practices.
Regularly communicate and collaborate with VHS providers regarding new patient referrals.
Perform patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, and other settings, including telehealth.
Collaborate with attending physicians regarding patient care and treatment, and support the coordination of hospice referrals.
Provide the Hospice Medical Director with the required face-to-face encounter documentation.
Perform other duties as assigned by the Medical Director.
Qualifications for Nurse Practitioner:
MSN from an accredited nursing school required; current state licensure as a Registered Nurse and Nurse Practitioner in good standing required in the state of Virginia.
2–4 years of experience in a medical specialty involving care of patients diagnosed with serious, advanced, or terminal illnesses is required.
Minimum of 2 years of clinical experience in Oncology, Pain & Symptom Management, Palliative Care, Hospice, hospital, or physician practice setting required.
Current CPR Certification required.
Ability to obtain hospital privileges as required.
Certification by the American Academy of Nurse Practitioners and/or the National Board for Certification of Hospice and Palliative Nurses (NBCHPN) preferred; certification required within an agreed-upon timeframe if not currently obtained.
Valid driver’s license, personal vehicle for on-the-job transportation, and proof of insurance as required per policy.
