Job Overview
We are seeking a dedicated and detail-oriented Hospice Admissions Coordinator to join our compassionate healthcare team. The ideal candidate will possess a strong background in medical office operations, patient management, and administrative support within a healthcare setting. As a Hospice Admissions Coordinator, you will be responsible for managing the patient admission process, coordinating care transitions, and ensuring compliance with health regulations. Your role is vital in providing seamless service to patients and their families during sensitive times, fostering a supportive environment through effective communication and administrative excellence.
Responsible for managing the Hospice Bridge Program and Intake process of patients to Hospice Care, and serve as a liaison between staff, clients, healthcare providers, discharge coordinators, and referral sources. As needed, provide continued support in all other intake workflow and clinical support services procedures in other service lines.
- DUTIES & RESPONSIBILITIES
- Process all bridge and outside patient referrals in a timely manner.
- Thoroughly review and approve referrals, and ensure admission is appropriate based on company, payor source, state, and federal guidelines.
- Coordinate all aspects of the intake process including scheduling admission evaluations, and effective and efficient communications with the patient, their family/responsible party, hospital/discharge planner, and staff.
- Gather all required information and documents, and ensure they are complete for admission. Follow-up on orders with physicians and healthcare providers as needed.
- Create a medical chart accordingly.
- Initial review of CTI and approval of F2F documentation, and work with Medical Records and Audit Partners to ensure that documentation is within CMS guidelines.
- Collaborate with the team on the intake procedures for quality assurance and implementation of new policies/ work methods.
- Establish and maintain positive working relationships with current and potential referral sources including community liaisons and Marketing team.
- Oversight and management of referrals and review of coding in compliance with PDGM standards and expectations.
- Provide assistance in coordinating respite care for patients.
- Be a back-up in managing the Intake Supervisor Workflow and clinical reviews of patient referrals in other service lines and provide assistance with other support services including medical records, scheduling and intake calls.

- JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Registered nurse or Licensed Practical Nurse with current licensure to practice professional nursing in the state.
- At least three to five years of experience in health care support services.
- Critical-thinking and clinical decision-making skills
- Strong verbal and written communications skills, public relations skills, and organization skills.
- Excellent customer service skills.
- Ability to learn and utilize electronic medical record and computer systems effectively and efficiently.
- Ability to work independently, and demonstrate autonomy, organization, adaptability, and cooperation in performing job responsibilities.
- Must comply with State COVID Vaccination requirements.
Pay: $24.00 - $28.00 per hour
