Pembroke Medical Associates is looking to hire a nurse practitioner to collaborate with a post acute and long term care facility located Hingham, Ma.
Duties:
- Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests.
- Develop and implement patient care plans in collaboration with the healthcare team.
- Prescribe medications and treatments based on patient needs and condition.
- Provide education to patients and their families regarding health conditions, treatment plans, and preventive measures.
- Monitor and manage chronic conditions, such as diabetes or hypertension.
- Collaborate with other healthcare professionals to ensure coordinated and effective patient care.
- Maintain accurate and up-to-date medical records.
Experience:
- Minimum of 1 year of experience as a Nurse Practitioner in a primary care or acute care setting.
-Acute care experience highly preferred.
- Proficient in using electronic medical record (EMR) systems for documentation and communication.
- Knowledge of aseptic technique for infection control during procedures.
- Experience in patient assessment, diagnosis, and treatment planning.
- Experience in memory care for patients with cognitive impairments.

Qualifications:
- Valid Nurse Practitioner license in Massachusetts.
- Certification as a Nurse Practitioner in the relevant specialty area (e.g., Family Practice, Adult-Gerontology, Acute Care Nurse Practitioner).
- Current BLS certification.
