The ideal candidate for this Nurse Case Manager position is reliable and experienced in all aspects of resident care in a long-term care setting. The Nurse Case Manager plays a critical role in caring for 25-35 assisted living and memory care residents.
Responsibilities:
- Case manage and coordinate care for medical and holistic care services, outcomes, and quality metrics for 25-35 assisted living and memory care residents.
- Collaborate and coordinate with outside clinical community partners and internal interdisciplinary team to ensure continuity of care.
- Conduct health evaluations and implement appropriate interventions.
- Participate in all surveys and inspections conducted by authorized regulatory groups
- Adhere to HIPAA regulations and maintain confidentiality of all Protected Health Information.
- Participate in on-call nursing phone support (one week on, two weeks off). Rotating on-call pay is additional to base pay.
- Other work-related tasks as assigned.
Experience & Requirements:
- Current Idaho state LPN or RN license (or multistate compact license)
- At least 1 year practical experience in nursing with experience in elderly care preferred
- Good computer skills and experience using electronic health record systems
- Must pass criminal background check in accordance with state requirements
- Obtain/maintain current First Aid and/or CPR certification as required
- Must be in good physical health (able to intermittently and routinely sit, stand, walk, bend, rotate, and conduct other physical movements inherent to this position and able to occasionally lift up to 50 pounds for a full 8-hour shift) and demonstrate emotional stability
- Must possess the ability to appropriately make independent decisions (within practice scope)
- Ability and willingness to deal tactfully and harmoniously with professional and non-professional personnel, residents, family members, visitors, government agencies/personnel, and the general public
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to practice continuous resident advocacy
- Willing to seek out new methods and principles and incorporate them into existing practices
- Ability to function independently, have flexibility, personal integrity and knowledge to work effectively with residents, family members, staff, and other support agencies
Wage: $40 - $48 / hr DOE. On-call support (by phone, not in person) paid in addition to regular wage.
