Job Description
Supervises: Care Pros/Med Techs
Reports to: Resident Care Director
Description: The primary purpose of your job position is to assist the Resident Care Director in planning, organizing, developing and directing the overall operation of the Resident Care Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility to ensure that the highest degree of quality care is maintained at all times.
Education: Nursing Degree from an accredited college or university.
Experience: Must have, as a minimum, three (3) years of experience as an LPN in a hospital, assisted living facility, or other related health care facility.
Specific Requirements:
Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in this state.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be able to read, write, speak, and understand the English language.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Prepare and maintain accurate medical records & documentation – reporting change of condition to physicians.
Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to assisted living communities.
Possess leadership and supervisory ability and possess effective communication skills.
Able to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Review all resident information and complete an assessment of the resident prior to move-in, the day of move in, 30 days after move in and following a change in condition.
Assist with family and resident concerns or problems and assist with resolving issues.
Professional and calm in emergency situations.
Administer medications and treatments according to physician’s orders and company policies.
Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs.
Manage daily resident care. Coordinate additional health services residents may need.
Assist in coordinating resident transfer to and from a hospital or another facility.
Be on call for medical questions and resident emergencies as scheduled.
Assist in coordinating residents’ dietary needs with the Food Service Director.
Be proficient in all documentation/record maintenance to ensure accuracy, resident confidentiality & compliance.
Assist with managing nursing budget including staffing hours and ordering supplies.
Physical Job Requirements
To fulfill job requirements, employee must be able to perform key physical task for extended periods of time. When supporting, lifting or transferring residents, proper body mechanics and safety measures as prescribed by the residence must be used.

In a typical Twelve - hour day, employee will:
- Stand/walk up to six hours a day
- Sit up to four hours a day
- Frequently support up to 70 pounds
- Occasionally lift/carry up to 15 pounds
- Frequently kneel, bend, and reach
- Secure proper assistance for transferring and lifting
of residents as needed
