Amore Enchanted Homecare Agency is seeking a dependable, skilled, and compassionate Licensed Practical Nurse (LPN) to join our homecare team. In this role, you will provide high-quality, continuous standard nursing care to individuals in the comfort of their homes.
The ideal candidate will have strong clinical skills, excellent observational abilities, and a commitment to maintaining patient safety, comfort, and independence.
Key Responsibilities
- Vital Signs & Assessments: Conduct regular health checks, including monitoring blood pressure, pulse, respiration, temperature, oxygen saturation, and routine health assessments.
- Medication Administration & Management: Administer oral, topical and injectable medications according to physician orders; manage medication schedules, refills and educate patients/families on compliance.
- Wound & Skin Care: Perform routine wound assessments, clean and dress surgical or pressure wounds, observe for early signs of infection, and implement preventive skin care measures.
- General Skilled Care: Perform routine enteral feeding (G-tube), catheter maintenance, tracheostomy care (if applicable), and infection-control best practices.
- Activities of Daily Living (ADLs): Provide hands-on assistance with personal hygiene, grooming, bathing, dressing, and mobility assistance as needed.
- Safety & Transfers: Practice safe patient handling techniques, utilize assistive transfer equipment (such as Hoyer lifts), and follow strict fall-prevention protocols.
- Documentation & Reporting: Maintain accurate, timely shift notes and medical logs; report any change in patient condition promptly to the supervising RN or primary physician; log time/visits via EVV systems (SwyftOps).
Qualifications
- Licensure: Active state LPN license in good standing.
- Certifications: Active CPR / BLS certification.
- Medical Screening: Current TB test (within the last 12 months) and ability to pass a background check.
- Experience: Minimum 1 year of clinical or home health nursing experience preferred.
- Skills: Strong physical assessment skills, medication management proficiency, and familiarity with assistive mobility devices.
- Key Attributes: Reliable, detail-oriented, patient-focused, and capable of working independently in a home environment.

- Pay Rate: Competitive hourly rate based on clinical experience.
