Medicare certified home healthcare agency looking for experienced and compassionate Physical Therapists to join our teams in multiple locations - Boston/Brighton/Dorchester/Mattapan/Hyde Park/Roxbury/JP/ Newton/Brookline/Allston
Metrowest Area- Framingham/Natick/Hopkinton/Holliston/Ashland
This is a full time/per diem position with flexible hours, visit work schedules and excellent reimbursement package. Our clinicians can choose their preferred geographical area to do home care visits.
- Provides initial and ongoing assessments to identify the client’s level of functioning and ability to communicate, including OASIS assessments at appropriate time points. (PT only)
- Develops and revises the plan of care in consultation with the physician and other care team members.
- Communication with all the physicians involved in the plan of care and other health care practitioners related to the current plan of care
- Prepares clinical and progress notes that are legible, accurate, and complete and submits them in a timely manner according to agency policy.
- Per diem position, Preferred Locations, Flexible Schedule
- PT rates -up to $150 per visit
- Required Licenses/Certifications:
- Current license as a PT in the Commonwealth of Massachusetts required
- Current CPR required
Additional Requirements:
- Prior experience in home health care preferred
- Valid state driver’s license
- Must have a reliable car with insurance coverage.
