Collaborative Team Culture
Administer medications safely and accurately.
Perform physician-ordered treatments, including wound care and respiratory treatments.
Monitor residents for changes in condition and report concerns promptly.
Collaborate with RNs, CNAs, therapists, physicians, and other interdisciplinary team members.
Maintain accurate electronic health records (EHR) and documentation.
Ensure compliance with all federal, state, and facility policies and regulations.
Supervise and support Certified Nursing Assistants (CNAs) during your shift.
Provide compassionate, resident-centered nursing care.
Perform other duties as assigned.
Active, unrestricted Wisconsin Licensed Practical Nurse (LPN) license required.
Minimum of 1+ years of experience as an LPN is required.
Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.
Strong clinical assessment and communication skills.
Experience using Electronic Health Records (EHR) preferred.
Ability to work independently and as part of a multidisciplinary team.
