Job Summary
ACMS Home Health Services, LLC is seeking a compassionate, skilled, and reliable Registered Nurse to provide high quality care to patients in their homes. The Registered Nurse is responsible for assessing patient needs, developing and implementing individualized Plans of Care, coordinating with physicians and interdisciplinary teams, and providing skilled nursing services in accordance with physician orders, agency policies, and state and federal regulations.
Key ResponsibilitiesPatient Care
- Perform comprehensive initial and ongoing patient assessments
- Develop, implement, and update individualized Plans of Care based on patient needs and physician orders
- Provide skilled nursing care including wound care, medication management, injections, catheter care, intravenous therapy, ostomy care, and disease management
- Monitor patient condition and report significant changes to the physician and Director of Nursing
- Educate patients and caregivers on diagnosis, medications, treatments, safety precautions, and disease management
- Assess pain levels and implement pain management interventions
- Perform medication reconciliation and monitor for side effects or adverse reactions
Care Coordination
- Coordinate patient care with physicians, therapists, home health aides, and other healthcare professionals
- Obtain and communicate physician orders for changes in treatment or condition
- Supervise Licensed Practical Nurses and Home Health Aides as required
- Participate in case conferences and interdisciplinary team meetings
- Ensure continuity of care and proper discharge planning
Documentation and Compliance
- Complete accurate, timely, and thorough documentation in the electronic medical record system such as WellSky or Kinnser
- Document assessments, interventions, patient progress, and communication notes according to agency policy
- Ensure compliance with Medicare Conditions of Participation, Florida Agency for Health Care Administration regulations, Community Health Accreditation Partner standards, and agency policies
- Maintain patient confidentiality and compliance with the Health Insurance Portability and Accountability Act
- Complete Outcome and Assessment Information Set assessments including Start of Care, Recertification, Transfer, Resumption of Care, and Discharge when required
Quality and Safety
- Assess home environment for safety risks and implement fall prevention strategies
- Monitor infection control practices and educate patients and caregivers
- Report incidents, accidents, or abuse and neglect concerns according to agency policy
- Participate in quality assurance and performance improvement activities
Qualifications
- Active Florida Registered Nurse license in good standing
- Minimum of one year nursing experience with home health experience preferred
- Experience with Outcome and Assessment Information Set preferred
- Current Cardiopulmonary Resuscitation certification
- Valid driver’s license and reliable transportation with insurance
- Ability to travel to patient homes within assigned territory
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Bilingual in English and Spanish preferred

Pay: $17.00 - $55.00 per hour
Work Location: Hybrid remote in Miami, FL 33186
