Position Overview
The Physical Therapist is responsible for evaluating participants, creating individualized treatment plans, providing therapeutic interventions, and collaborating with the interdisciplinary team to optimize functional mobility, safety, and independence.
Key Responsibilities
- Perform evaluations to determine participants’ physical therapy needs.
- Develop and implement individualized care plans tailored to each participant’s goals.
- Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
- Educate participants and caregivers on safe mobility techniques and home exercise programs.
- Work closely with the interdisciplinary team to ensure coordinated, person-centered care.
- Document assessments, treatments, progress notes, and plans of care in accordance with PACE and regulatory requirements.
Qualifications
- Licensed Physical Therapist (PT) in the state of North Dakota (or ability to obtain licensure).
- Experience with geriatric populations strongly preferred.
- CPR certification required.
- Strong communication, teamwork, and organizational skills.
- A passion for working with seniors in a community-based setting.
