As a Care Coordinator, you'll play a vital role in ensuring our clients receive compassionate, reliable care while supporting caregivers and collaborating with our internal team. What You'll Do
- Coordinate the start of care for new clients and ensure a smooth onboarding process.
- Maintain accurate client and caregiver records in our software systems.
- Communicate with clients, families, caregivers, and healthcare professionals regarding care needs and updates.
- Monitor client satisfaction through routine follow-up calls and quality assurance check-ins.
- Assist with caregiver-client matching based on client needs and caregiver qualifications.
- Track required documentation and ensure compliance with company policies and state regulations.
- Support caregivers with questions, concerns, and ongoing communication.
- Collaborate with office staff to ensure continuity of care and excellent customer service.
- Assist with care plan updates and documentation as needed.
What We're Looking For
- Previous experience in home care, healthcare coordination, case management, or a related field preferred.
- Excellent communication and interpersonal skills.
- Strong organizational skills with exceptional attention to detail.
- Ability to prioritize multiple tasks in a fast-paced office environment.
- Proficiency with Microsoft Office and the ability to learn new software systems.
- Professional, compassionate, and customer-service oriented.
- High school diploma or equivalent required; additional healthcare or administrative experience preferred.
