St. Joseph's a Villa Center is seeking a Wound Care Nurse LPN to join their team! The Wound Care Nurse LPN provides each resident with routine daily nursing care and services in accordance with each resident’s assessments and care plan and as may be directed by management. Wound Care Nurse LPN Duties and Responsibilities:
Works with patients who have altered skin integrity.
Directs the day-to-day functions of the nursing assistants in accordance with current rules, regulations and guidelines that govern the long-term care facility.
Evaluates the skin surface of at-risk patients such as bed ridden elderly individuals to prevent decubitus ulcers.
Provides treatment protocols to ward off skin irritation and infection for wounds that may not heal well, because of other medical problems, such as diabetes or poor circulation.
Cooperates with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.
Provides direct nursing care as necessary and as department needs may dictate.
Develops and participates in programs designed for in-service education, on the job training and orientation classes for newly assigned personnel.
Implements standards of nursing practice in infection prevention reviewing and revising as necessary to reflect changes in nursing practice and resident population.
Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required.
Prepares and administers medications and performs treatments as ordered by the physician.
Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.
Completes the required forms upon admission, transfer, and/or discharge.
Makes written and oral reports/recommendations concerning the activities of shift as required.
All other duties, as assigned.
Wound Care Nurse LPN Skills and Abilities:
Excellent written and verbal communication skills in English.
Serves as the expert in Wound Care.
Ability to multi-task and remain calm under stress.
Analytical and problem-solving skills.
Ability to organize and prioritize work.
Understands and follows infection control standards, policies and procedures.
Must be able to adapt to a flexible work schedule and occasional overnight travel.
Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
Wound Care Nurse LPN Education and Experience:
Current CPR Certification required.
Possess a current license to practice in the State as a Licensed Practical Nurse.
1 year of experience preferred.
Long-Term care experience preferred.
Physical Requirements:
Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required.
Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
