Role Overview
Skilled Nurse Practitioners with 1+ years of nursing home experience are needed to provide primary care and wound care services across post‑acute and long‑term care facilities throughout Ohio.
Primary Care roles range from 3–5 days per week, and Wound Care roles are available 3 days per week in the Toledo region.
Primary Care Coverage
- Newark – 43055 — 5 days per week
- Defiance – 43512 — 4 days per week
- Marion – 43302 — 3 days per week
- Fremont – 43420 — 3 days per week
Wound Care Coverage
- Toledo Region — 3 days per week
Key Responsibilities
- Primary Care Management — Conduct comprehensive evaluations, manage chronic conditions, address acute changes in condition, and provide ongoing medical oversight for residents.
- Care Plan Oversight — Maintain individualized care plans, document clinical progress, and collaborate with interdisciplinary teams to ensure continuity of care.
- Medication Management — Review medications, reconcile orders, and ensure safe and appropriate administration practices.
- Wound Care Services — Conduct wound assessments, develop treatment plans, perform NP‑appropriate procedures such as debridement, and monitor for signs of infection.
- Interdisciplinary Collaboration — Work closely with nursing, therapy, dietary, and administrative teams to support healing and optimize resident outcomes.
- Family & Staff Communication — Provide clear updates, education, and support to residents, families, and facility staff.
- Regulatory Documentation — Maintain accurate, compliant clinical documentation for both primary care and wound care services.
Required Qualifications
- Active Nurse Practitioner license in Ohio with Board Certification.
- Active DEA preferred.
- 1+ year of nursing home or long‑term care experience.
- Strong clinical judgment and comfort working independently.
- Excellent communication and collaboration skills with DONs and nursing staff.
