Hometown Physician Services is a provider of in-home primary care to patients residing in assisted living facilities and long term senior care communities throughout areas in Wisconsin, and Minnesota. Immediate part-time Nurse Practitioner positions open (able to grow into FT in the future, if desired); with long term upward mobility and management opportunities, possible. We are seeking to fill a Nurse Practitioner position to help lead a care team and provide on-site primary care to patients in their long term care homes. This is a unique opportunity for a Nurse Practitioner to help lead and guide care team plans, clinical decisions and broader strategies, alongside involved collaboration & support from the clinical support team. Candidates should have interests in working with patients (especially seniors) with a range of physical and behavioral health needs and disabilities. Desired Talents and Qualifications:
- Graduate of NP program from an accredited school;
- Must have and currently maintain valid Nurse Practitioner license and DEA certification in Wisconsin (and MN is a plus, but not required);
- Experience in primary care and proficient EMR knowledge is preferred; geriatrics and in-home care a plus;
- Proven ability to communicate and collaborate effectively with colleagues, patients and families;
- Strong interpersonal and customer service skills, in particular with seniors and disabled individuals;
- Must have a valid driver’s license and minimum state required liability auto insurance; reasonable travel to & from facilities is required;
- Demonstrated compatibility with Hometown's mission and philosophies to serve our communities' elderly and disabled individuals.
