Healthcare Resource Specialist Member Position Description (half-time)
AMERICORPS PROGRAM OVERVIEW
Dan River Year AmeriCorps will partner with the REACH (Regional Engagement to Advance Community Health) program, a program of the Institute for Advanced Learning and Research (IALR), dedicated to expanding capacity for public health service by providing direct outreach and education service in the City of Danville and Pittsylvania County. Members will provide individualized support, addressing the region’s most pressing physical and mental health challenges.

POSITION SUMMARY
The Healthcare Resource Specialist is a key member of the REACH Partnership within the Advanced Learning Department. This position is responsible for supporting comprehensive care management by facilitating annual Social Determinants of Health (SDOH) screenings, connecting clients to appropriate community resources, and collaborating with community partners and health worker teams. The Specialist will also deliver educational workshops in targeted census tracts within Danville City and Pittsylvania County, with a focus on chronic health conditions and identified population health needs. This is a nine-month service role requiring approximately 30 hours per week, with opportunities for additional training and continued service upon successful completion.

MEMBER DUTIES
The Healthcare Resource Specialist will perform the following duties:
Client Support and Navigation
- Conduct annual SDOH screenings with clients and document results in the electronic health record (EHR).
- Identify needs and connect clients with appropriate community resources, including referrals to REACH Community Health Workers.
- Assist clients with navigating community services related to transportation, housing, food, childcare, and other social needs.
- Advocate for clients to support holistic health and equitable access to care.
Resource Management and Documentation
- Maintain accurate and up-to-date community resource guides for the REACH Partnership service area.
- Document screenings, follow-up calls, and client progress in the EHR and program records.
- Create and manage telephone encounters (TEs) for clients in need of resources.
- Submit regular reports and service documentation to the Program Coordinator.
Workshops and Community Outreach
- Facilitate a minimum of one weekly workshop in targeted census tracts in Danville City and Pittsylvania County, focusing on chronic health issues and priority population health topics.
- Obtain evidence-based training certifications to deliver workshops effectively.
- Support outreach events and collaborate on community service projects in the area of healthy futures.
- Recruit volunteers to assist with identified service opportunities.
Collaboration and Team Support
- Work closely with community partners, health worker teams, and IALR staff.
- Encourage open communication, feedback, and skill development within the team.
Program Participation
- Complete required trainings as outlined by the REACH Program Manager.
- Attend all mandatory host site, REACH Partnership, and AmeriCorps meetings.
- Accurately track and submit AmeriCorps service hours.
- Participate in required REACH AmeriCorps service projects.
APPLICANTS MUST:
- Be at least 18 years old on their first day of service
- Be a U.S. citizen or lawful permanent resident alien
- Have a high school diploma or high school equivalency or agree to obtain one during the term of service
- Not have served more than the allowed number of AmeriCorps terms previous to this term
- Dress appropriately; i.e., as a professional educator and wear AmeriCorps apparel or button while in service.
- Submit to a full background check, including a sex offender registry check, state criminal history registry check, and FBI fingerprint-based check. This position may have recurring access to vulnerable populations and selection is contingent upon satisfactory results of these checks.
PREFERRED QUALIFICATIONS:
- Strong oral and written communication skills
- Strong health proficiency
- Health background or professional experience working with public
- Associate’s degree or higher
- Silver level National Career Readiness Certificate
- Intermediate computer skills, specifically in Microsoft Office products
- Ability to work independently and as part of a team
- Access to a reliable vehicle and valid driver’s license
- Strong organizational skills
- Technology literate with the ability to use virtual meeting platforms and online programs
BENEFITS:
· Living allowance of $13,500 for a service term of 900 hours over 9 months
· Eligibility for Segal Education Award of $3,697.50 at the end of a successful term of service, which can be used to repay student loans or for current or future education.
· Mileage reimbursement for non-host site location travel

Pay: $1,500.00 per month
