As a Case Manager, you will be tasked with assessing, planning, coordinating, implementing, monitoring, and evaluating the services required to meet the diverse needs of clients. You will work closely with individuals, families, or specific groups, advocating on their behalf to ensure access to necessary resources and services. This role involves collaborating with interdisciplinary teams, maintaining accurate records, and continuously evaluating and adjusting care plans to address changing needs.
Responsibilities:
- Assessment: Conduct comprehensive assessments of clients' needs, strengths, and challenges to develop personalized care plans.
- Planning: Develop individualized care plans in collaboration with clients, their families, and relevant professionals to address identified needs and goals.
- Coordination: Coordinate and facilitate access to services, resources, and community-based programs that align with clients' care plans.
- Advocacy: Advocate for clients' rights and needs within healthcare, social service systems, or legal entities as necessary.
- Monitoring and Evaluation: Regularly review and evaluate clients' progress, adjusting care plans as needed to ensure optimal outcomes.
- Documentation: Maintain accurate and detailed records of client interactions, services provided, and progress achieved, ensuring compliance with confidentiality and regulatory requirements.
- Collaboration: Work collaboratively with interdisciplinary teams, including social workers, healthcare providers, counselors, and community organizations, to ensure comprehensive support for clients.
- Crisis Intervention: Provide immediate support and intervention in crisis situations, ensuring clients' safety and well-being.
- Education and Support: Offer guidance, education, and emotional support to clients and their families, empowering them to navigate services and resources effectively.
Qualifications:
- Strong administrative skills.
- Knowledge of FMLA paperwork and disability a plus
- Relevant experience in case management or related fields may be required.
- Knowledge of social services, healthcare systems, community resources, and relevant laws and regulations.
- Strong communication, interpersonal, and organizational skills.
- Empathy, cultural competence, and the ability to work effectively with diverse populations.
- Proficiency in maintaining confidentiality and ethical standards in client care.
Job Type: Full-time
Pay: $24.00 - $26.00 per hour
