compassionate, quality care to patients in their homes.
Job Description
- Perform initial and ongoing patient assessments (including OASIS) and develop individualized care plans
- Administer medications and treatments per physician orders
- Perform wound care, including cleaning and re-dressing as needed
- Measure and monitor vital signs, and identify/report changes in patient condition
- Coordinate with physicians, LVNs, HHAs, and other care team members on treatment plans
- Supervise and delegate tasks to LVNs and HHAs within their scope of practice
- Maintain accurate, compliant clinical documentation and progress reports
Qualifications
- Strong initiative to keep the agency in compliance
- Well organized, self-motivated, and able to work independently under the supervision of the Director of
Nursing
- Excellent organization, communication, and follow-up skills
- Excellent computer skills (EMR documentation experience preferred)
Requirements
- Valid CA RN license
- Valid CA drivers license
Grow With Us
RNs at Get Well Home Health have a clear path to grow into leadership. Field RNs can move into a part-time QA
RN role, and from there into a full-time QA RN position overseeing chart review and compliance.
Pay: $35,000.00 - $100,000.00 per year
