Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately
- Administer medications and manage medical documentation in compliance with home healthcare regulations and HIPAA standards
- Support patients with medical procedures such as catheter care, wound care, and end-of-life care as directed by healthcare providers
- Operate patient transfer methods safely using equipment like Hoyer lifts or transfer belts to ensure patient safety during mobility tasks
- Maintain patient confidentiality while communicating effectively with clients, families, and healthcare teams
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for patients
- Document all patient interactions thoroughly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Experience
- Prior experience working with individuals with disabilities or in assisted living environments is highly desirable
- Demonstrated experience in geriatrics or working with geriatric patients preferred
- Knowledge of medical terminology, medication administration, and basic life support (BLS) or CPR certification required
- Experience providing dementia or Alzheimer’s care, memory care or hospice support is a plus
- Familiarity with home health agency operations and home & community care regulations is advantageous
- Proven ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or patient transfer devices
- Strong communication skills for effective client interaction and team collaboration
