About Us:
We are a network of substance abuse clinics serving clients in the Baltimore City and County area with six locations. We are currently seeking a Registered Nurse to join us as a Health Home Care Coordinator, working collaboratively with our team to provide exceptional care for our patients.
Responsibilities:
- Coordinate care for assigned participants.
- Ensure implementation of treatment plans in partnership with individuals and families.
- Conduct health promotion and education.
- Provide referrals and additional supports as needed.
Principal Responsibilities and Duties:
- Develop wellness and prevention initiatives.
- Deliver health education to participants and their families/caretakers.
- Participate in developing initial treatment plans for Health Home enrollees.
- Assist in setting goals for individuals with chronic diseases or at risk for co-occurring conditions.
- Collaborate with community resources to address social and medical issues.
- Coordinate with medical providers and hospitals for admissions and discharges.
- Track required assessments, screenings, and immunizations.
- Monitor HIT tools and reports for treatment updates, medication alerts, and hospital activities.
Requirements:
- Active RN license to practice in Maryland
