The Care and Service Coordination Supervisor is responsible for the direct supervision of Case Managers who provide service for individuals over 60 or individuals under 60 and enrolled in Choices for Care; assists with integration of the Care and Service program with other Age Well programs, and assists with the training, mentoring and evaluation of the Case Managers. They will work closely with the Director of Care and Service Coordination to maintain the quality of the Care and Service Coordination Program. Will provide backup assistance to Case Managers as needed.
This position is located in our St. Albans office.
The successful candidate will have a BA/BS Degree (Masters preferred), along with five years of experience in a related field including demonstrated capacity for supervision of others, and direct service experience.
