Inclusion Care Solutions is a mission-driven Rural Health Clinic dedicated to providing comprehensive, person-centered care to underserved populations, including individuals with intellectual and developmental disabilities (I/DD). We are building a fully integrated care model that includes primary care, behavioral health, and care coordination—ensuring access, dignity, and lifelong support from diagnosis through end-of-life.
We are seeking a compassionate, motivated Nurse Practitioner (NP) to join our growing clinical team. This provider will play a critical role in delivering high-quality primary care services in a rural setting while helping shape an innovative model of care for vulnerable populations.

Provide comprehensive primary care services across the lifespan

Conduct physical exams, assessments, and diagnostic evaluations

Diagnose and treat acute and chronic conditions

Prescribe medications and manage treatment plans

Provide preventative care, screenings, and patient education

Collaborate with interdisciplinary teams including behavioral health and case management

Maintain accurate and timely documentation in the EMR

Ensure compliance with all state, federal, and Rural Health Clinic regulations

Participate in quality improvement and care coordination initiatives

Requirements:

Active and unrestricted Kentucky APRN license
-
Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP

DEA license (or ability to obtain)

Experience in primary care, rural health, or underserved populations preferred

Experience with individuals with I/DD is a strong plus
