Deliver impactful care to patients in their homes through flexible, per diem work as a Nurse Practitioner focused on home health risk assessments. Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members in Salem and throughout Columbiana County
- Evaluate patients’ prior and current medical histories, medical conditions, and medication regimens during each visit, averaging 45 minutes per assessment
- Educate patients about their health and medications, fostering a deeper understanding of their conditions and treatment
- Identify gaps in care and provide clear recommendations to improve patient outcomes
- Visits may involve annual health assessments or post-discharge evaluations
- No requirement for prescribing medication or forming treatment plans, allowing you to focus on assessment and education
Qualifications:
- Current Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience is preferred
