Case manager is responsible for providing, coordinating, monitoring, and evaluating treatment plans and their progress; facilitating interdisciplinary approaches; and monitoring progress for patients covered under worker’s compensation. Work closely with physicians, nurses, mental health, ER staff, and other health care professionals, as well as family of patients. Maintains client's electronic file. Makes appropriate referrals for patients to obtain needed services. Creates various reports. Extensive documentation is necessary to ensure that the individual receives needed services and makes progress towards goals. This position includes field case management and some telephonic case management. You must reside near Seattle, WA to be considered for this position. This position includes report writing and summarizing documents. On call after regular hours may be required from time to time.

· Ability to analyze complex situations and recommend appropriate courses of action.
- Strong verbal and written communication skills. Must be able to write cohesive and comprehensive documentation. Bilingual English and Spanish is preferred.
- Ability to handle confidential and sensitive information in a professional manner with discretion and integrity. Understanding of HIPAA rules and compliance.
- Ability to establish and maintain effective working relationships with clients, staff, community members and other stakeholders.

Background Experience:
· Case Management experience required
· Registered nurse or Bachelor’s degree from an accredited college or university with a major in a Nursing, Social, Behavioral Health or Human Service area. Salary is commensurate with education and experience
- Ability to travel within a designated geographic area for in-person case management activities
- Excellent analytical and problem-solving skills
- Effective computer skills including navigating multiple systems and keyboarding
Preferred Skills/Experience:
- Bilingual preferred
- 3+ years of case management experience
- Ability to work independently
- Additional national professional certification (CRC, CDMS, or CCM)
- Knowledge of laws and regulations governing delivery of rehabilitation services, including HIPPA
- Effective communications, organizational, and interpersonal skills.
- Job-specific technical knowledge (e.g., knowledge of workers compensation and disability industry for workers’ compensation case managers or case management)
- Bachelor's and/or Master's degree in Nursing or related field
