Job Summary
We are seeking a dedicated and compassionate Psychiatric Rehabilitation Professional (PRP) / Community Support Worker (CSW) to join our dynamic behavioral health team. Duties
- Assist clients with daily living activities, including personal hygiene, medication management, and vital signs monitoring to promote health and safety.
- Implement individualized treatment plans utilizing applied behavior analysis (ABA), behavior management techniques, and crisis intervention strategies to support behavioral improvements.
- Conduct intake assessments, collect data on client progress, and maintain accurate medical documentation in compliance with HIPAA regulations.
- Provide therapeutic support tailored to clients with diverse needs such as autism spectrum disorder (ASD), PTSD, developmental disabilities, or addiction issues.
- Collaborate with multidisciplinary teams including social workers, medical professionals, and educators to coordinate care and ensure holistic support.
- Engage in crisis management by de-escalating situations involving behavioral or emotional distress while ensuring safety for clients and staff.
- Support clients during community outings or social activities, fostering social skills development and community integration.
- Develop and maintain referral partnerships with healthcare providers, community organizations, schools, shelters, and behavioral health professionals.
- Conduct outreach, presentations, networking events, and marketing activities to promote PRP services and increase community awareness.
- Educate referral sources on PRP eligibility and benefits, while assisting prospective consumers through the referral and intake process.
- Track outreach activities, maintain referral records, prepare reports, and collaborate with PRP staff to ensure timely service delivery.
Basic Qualifications:
- High School Diploma or GED
- Strong communication and interpersonal skills
- Reliable transportation
- Valid driver's license
- Ability to pass required background screening
Pay: $30.00 - $65.00 per hour
