Position Overview
The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and delivering skilled therapy services to improve mobility, function, safety, and quality of life. This role primarily serves residents in assisted living communities and home-based outpatient settings.
Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized plans of care.
- Implement evidence-based treatment interventions to improve strength, balance, mobility, endurance, and overall function.
- Assess patient progress and modify treatment plans as clinically appropriate.
- Educate patients, families, and caregivers on exercises, safety strategies, and fall prevention.
- Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and community staff to optimize patient outcomes.
- Complete documentation accurately and timely in accordance with company policies and payer requirements.
- Participate in care conferences, interdisciplinary meetings, and team development initiatives.
- Maintain compliance with all federal, state, and professional practice standards.
QualificationsRequired
- Current New Jersey Physical Therapist license in good standing.
- Graduate of an accredited Physical Therapy program.
- Strong communication, organizational, and clinical reasoning skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
Preferred
- Experience working with geriatric populations.
- Assisted living, home care, outpatient, SNF, or long-term care experience.
- Experience with Net Health or electronic medical record systems.
