Qualifications
- Current Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification, as required by state regulations.
- Minimum of one (1) year of experience providing direct patient care preferred.
- Experience in home health, long-term care, assisted living, or hospital settings is a plus.
- Ability to communicate effectively with patients, family members, and healthcare professionals.
- Reliable transportation and willingness to travel to patient homes within the assigned service area.
Position Summary
The Home Health Aide/Certified Nursing Assistant provides personal care and supportive services to patients in their homes under the direction of a Registered Nurse or other qualified healthcare professional. The aide assists patients with daily living activities while promoting comfort, dignity, independence, and safety.
Essential Duties and Responsibilities
The Home Health Aide/Certified Nursing Assistant will perform duties including, but not limited to:
- Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and personal hygiene.
- Provide assistance with ambulation, transfers, positioning, and mobility exercises.
- Support patients with meal preparation, feeding assistance, and maintaining adequate hydration.
- Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.
- Assist with routine household tasks related to patient care, including light housekeeping and laundry.
- Help maintain a clean, safe, and comfortable home environment.
- Reinforce patient care instructions as directed by the clinical team.
- Document services provided and patient observations accurately and promptly.
- Maintain professional boundaries while building positive relationships with patients and families.
- Follow individualized care plans developed by the supervising clinician.
- Participate in supervisory visits and team meetings as required.
- Comply with infection control practices, patient confidentiality standards, and agency policies.
- Provide compassionate care that respects each patient's preferences, culture, and personal dignity.
- Adhere to all state, federal, Medicare, Medicaid, and agency regulations governing home health services.
