First To Serve, Inc. (FTS), a residential housing program for men and women is seeking a highly qualified Licensed Vocational Nurse (LVN ) to assist the management with primarily with medication management, mental health, substance abuse, domestic violence, and homelessness.
Responsibilities:
General Responsibilities:
- To perform the full range of first level nursing; including daily routine wellness checks, medication compliance, record keeping and general oversight of any and all medically necessitated issues presented
- To always portray a positive attitude in communicating, interacting and in the provision of services to residents and their families, including other staff members and visitors
- To demonstrate initiative, flexibility and a willingness to adapt to changing situations and changes in flow, up to and including working weekends
Resident Admission responsibilities:
- Compare resident's ODR medication records with the medications resident arrives with
- Make sure resident has all their medications and each medication is for 3-30/31 day supply
- Initiate the ADMISSION INVENTORY LOG, MONTHLY MED CALL LOG, MEDICATION INVENTORY LOG, MEDICATION REFILL LOG for each resident and place under resident's name in each log book
- Notify referring provider upon onset of any discrepancy and follow up the next day
- If no medications from referring provider arrive on day (2) of patient admission, notify MD/NP
- Document in the ADMISSION INVENTORY LOG phone numbers to resident's referring provider, case manager (i.e. Amity, LAMP, Project 180, VOA), resident's medical provider, mental health provider, Nurse Practitioner and/or psychiatric provider
Medication Responsibilities
- Set up, monitor and ensure resident's medication taking for morning, afternoon and evening dosages while allowing flexibility for residents who are not on time
- Ensure that each resident is identified and taking the correct medication and dosage
- Attempt to locate resident's who do not attend med call in the facility and encourage them to come for their medications
- Be able to administer intramuscular injections to residents as scheduled and necessitated; after having been checked off by Nurse Practitioner in intramuscular medication administration
- Monitor and identify life threatening side effects of medications and the appropriate interventions
- Report identified side effects of medications immediately to the MD/NP
- Maintain a monthly MED CALL LOG on each resident
- Ensure that staff and the resident's sign the "monthly med call" log immediately upon taking or ingestion of the assigned medication/dosage
- Notify resident's FTS case manager and their referring provider (i.e. Amity, LAMP, Project 180, VOA) whenever resident is non-compliant with medications, by day (2) of missing med call
- For each resident, document on Mondays, Wednesdays and Fridays of each week the number of medications left per resident prescription in the MEDICATION INVENTORY LOG
- Complete and forward to resident's referring provider (i.e. Amity, Project 180, LAMP, VOA) MEDICATION REFILL LOG (7) days before patient's medications are finished
Wellness check responsibilities:
- Provide information on resident's medications, diet, exercise and other topics deemed appropriate by FTS and NP/MD
- Identify other necessary educational topics and report to MD/NP
- Perform wellness checks (Vital signs monitoring)
- Assist residents in a culturally, linguistically, and developmentally appropriate manner
- Use active listening and motivational skills during resident interactions
- Identify and escalate calls to Physician or Nurse Practitioner for emergency services, as deemed necessary.
Documentation:
- Document in patient progress notes, all consequential interactions involving resident and staff
- Document any reports made to providers, MD/NP on behalf of the resident
Other Responsibilities:
- Other functions deemed necessary for the proper running of FTS.
- Fluent in writing and speaking Spanish; Bilingual preferred
