Position Overview
As a Home Health Physical Therapist, you will:
Provide individualized, one-on-one therapy services in patients’ homes

Evaluate patients and develop customized treatment plans
-
Monitor patient progress and adjust interventions as needed
-
Educate patients and caregivers to maximize outcomes

Collaborate with physicians and interdisciplinary healthcare teams
-
Qualifications
Doctorate or Degree in Physical Therapy (DPT or equivalent)

- Minimum 2 years of clinical Physical Therapy experience
Home Health experience preferred

Active California Physical Therapist License
-
Valid California Driver’s License
-
Current CPR certification
-
Current TB test and Professional Liability Insurance

Strong communication, documentation, and organizational skills

Ability to work independently and manage a flexible caseload
